    You will participate as a member of the PACE Interdisciplinary Team in the care of high-risk older adults living in the community with a strong focus on the participants' goals as well as excellent geriatric care management.
    FLEXIBLE SCHEDULING: Monday-Friday scheduling, with a possibility for a 32-hour workweek or 40-hour work-week with 1 day remote. To be discussed during the interview process.
    QUALIFICATIONS
    -Must be comfortable with the aging and elderly patient population
    - Quickly recognize situations/problems before becoming acute, implement effective solutions, and understand consequences
    -Board certified in IM or FM. Board certification in Geriatric Medicine is PREFERRED
    -Bilingual in Spanish highly regarded!
    - Current MA license, DEA prescription privileges, and CPR Certification
