  • Additional Info: Perform duties in the Department of Homeless Services (DHS) medical clinic located at Prevention Assistance and Temporary Housing (PATH). Each NP will have rooms in which to provide assessments and examinations:
    -Provide medical assessments and acute care services to the Department of Homeless
    Services pediatric, adolescent, and adult applicants
    -Provide and document adequate assessment details and relevant patient history
    -Complete medical triage health screening forms via CARES
    -Provide counseling for Nicotine Replacement Therapy "NRT" and Safe Sleeping "SIDS
    -Use appropriate and approved reference material, up to date, to provide relevant pediatric and
    adult patient care at PATH
    -Collaborate with Infection Control
    Committee in compliance
